  per group (two teams)

• It is our intention to have a player to coach ratio of
  6 to 1

I, hereby accept the terms of this application and registration and hereby release, remise and forever discharge the OKPrep Camps, and its directors or agents and all persons associated with its firm and all liability whatsoever or responsibility whatsoever for accidents or injuries, whether fatal or otherwise, which may occur incidentally to or arising directly or indirectly out of OKPrep Camps.

This is also my written permission to have myself/my child admitted and attended to for medical or dental treatment in case of sickness or injury.  This release shall be binding on the players, heirs, assigned executors and administrators.

REFUND POLICY OF OKPREP CAMPS:

Prior to June 1st Full Refund less administration fee of $75.00
After June 1st there will be no money refunded however, OKPREP will reserve a spot in the following year for the cancelled player The cancellation can be for any reason or injury. (No explanation necessary)

Injury during OKPREP Camps: No refunds unless you have our "Injury Insurance" We offer injury insurance for $25.00 per session that will cover the player if he/she is injured during our camp. The refund will be based on a pro-rated basis.

Reasons that a player will be asked to leave OKPREP Camps (With no refund)

1. Theft (RCMP will be involved)
2. Illegal Drugs (RCMP will be involved)
3. Bullying or Violence
4. Vandalism

We will have younger hockey players attending our camp and will demand that the older players be positive role models.

Payment
*All prices are in Canadian funds*

*OKPREP Camps willing to accept post dated
cheques up until July 15th.  After that date the
cheque must be certified.*

*A $150 dollar deposit is required to hold your spot.
Balance is due on June 1 2008.*

If registering after June 1st 2008,
full payment is required to hold your spot
